The Paul Gough Show
EP686 It’s Not Your Income. It’s Your Identity. (Live From PPM)
In this episode, I’m pulling no punches.
I’m sharing the raw, unfiltered truth about what it’s really like to grow a successful private physio clinic in the UK — especially when you’re up against the so-called “free” NHS, GP indifference, and a public that doesn’t know you exist.
When I started my clinic back in 2007 in Hartlepool — one of the poorest towns in the UK — I faced every obstacle you can imagine. No doctor referrals. No money in the town. Everyone brainwashed to believe physio should be free. And yet, I still built a cash-pay practice that scaled to four locations, brought in over £60K a month, and served more than 1,000 visits monthly.
How did I do it?
Not by being the best physio.
I did it by becoming a world-class marketer of my physio business.
In this episode, I’ll show you:
✔️ Why you can stop chasing GP referrals — and what to do instead
✔️ How to compete (and win) against the NHS
✔️ Why your marketing, not your methods, determines your success
✔️ How your website, social media, and systems can transform your business
✔️ And why NOW is the best time to build a real marketing machine in your clinic
If you're feeling invisible in your community, struggling to get enough patients, or just plain tired of working hard with little to show for it — this episode is for you.
This isn’t a training.
It’s not a webinar.
And it’s definitely not three tips to fix your marketing.
In this episode of Coffee with Goughy, I go straight to the real reason so many clinic owners feel stuck, overwhelmed, or quietly frustrated, even when the business looks “fine” from the outside.
I talk about leadership.
Standards.
Insecurity.
Delegation.
Pricing guilt.
And the hard truth most owners don’t want to admit: if a problem has been hanging around long enough, it probably isn’t a staff issue. It’s a leadership one.
This is a raw, unfiltered conversation about what actually limits growth in a clinic, why businesses flatline at certain revenue levels, and how your confidence, decisions, and tolerance for discomfort determine how far your business can really go.
No tactics.
No fluff.
Just an honest conversation over coffee about what it takes to build a clinic that works without breaking you.
Grab your coffee and listen in.

817. EP 817 ☕️ "Should Staff PT's Be Forced To Wear Your Clinic Uniform?" [Coffee with Goughy]
01:05:00||Ep. 817Today, I break down three real-world business stories — Starbucks uniforms, Uber layoffs, and UK work-from-home laws — and what they reveal about the fight every clinic owner is now facing.Explains why employee "identity" demands (uniforms, individualism) are colliding with what businesses actually need to survive, using the Starbucks dress-code controversy and the Ed Sheeran opening-act backlash as case studiesBreaks down the "grow and prune" principle behind Uber's layoffs-despite-record-profits story, and argues most PT clinics are overstaffed and under-profitable as a result of avoiding hard staffing decisions
816. EP 816 ☕️ "6 Successful PT Clinic Owners Tell The Truth About Building A Private Practice In 2026" [Coffee with Goughy]
35:55||Ep. 816Today, six successful PT clinic owners divulge the mistakes, beliefs, and hard-won lessons that shaped their businesses.Reveals candid confessions on firing top-producing toxic employees, going nearly six figures into debt, and the marketing tactics (newspaper ads, $150K salaries, sales scripts) they once thought were bullshit, but now swear byUncovers why revenue without profit almost sank several of their businesses, and how community — not information — became the real driver of their growth
815. EP 815 ☕️ "Is The PT Profession In Crisis? Your Questions Answered!" [Coffee with Goughy]
01:12:36||Ep. 815In Today's Episode, I Answer These Questions:Why are PT clinics generating more revenue than ever while owners are taking home less money? How can a $2 million clinic be a worse business than a $1 million clinicWhat is a respectable profit margin for a PT clinic in 2026—and how vulnerable is an owner operating at just 5–10%?Why should clinic owners pay themselves a $150,000 salary before calculating profit? Are insurance companies really the problem, or are clinic owners using them as an excuse to maintain an outdated business model? How vulnerable is a clinic that still relies primarily on doctors and word of mouth for new patients? Why are clinic owners posting constantly on social media but still failing to generate predictable new patients? Is the traditional insurance-based PT clinic genuinely dying—or is that just a provocative claim? Is the inability to pay a great PT $150,000 actually a staffing problem—or a profitability problem? Why can one clinic pay exceptional PTs well and still make a 25% profit while another cannot afford an additional $10,000 in salary? Is it true that nobody wants to work anymore—or have the expectations of employees and business owners simply changed? When does a difficult employee become a toxic employee, and how much can keeping the wrong person cost your clinic? Why do otherwise successful clinic owners tolerate toxic staff for so long? What should you do when your highest-producing PT is also the person poisoning your culture? Will AI genuinely allow PT clinics to operate with fewer employees—or is it all just industry hype?
814. EP 814 ☕️ "What is One Thing a Staff PT Has Said That Pissed You Off?" [Coffee with Goughy]
01:13:28||Ep. 814Today, I break down the staff comments that are quietly driving clinic owners to their breaking point — and what's really behind them.Reveals the real pattern behind entitled staff attitudes, and why it's not insurance or the economy that's killing your clinic's growthBreaks down why insecurity — in owners and employees alike — is the hidden root of nearly every "my staff PT pissed me off" story you've ever had
813. EP 813 🔥 "Are You the Reason Your Patients Quit?" [Paul on Fire]
06:14||Ep. 813Today, I break down why patients quietly quit on their PTs, and how giving out too many exercises can be doing more harm than good.Revealing a blind spot most clinic owners never catch until it's too lateBreaking down the simple shift that separates practices patients stay loyal to from the ones they quietly leave
812. EP 812 🔥 "What Are You Holding Onto That Creates A Bigger Headache Elsewhere?" [Paul on Fire]
06:35||Ep. 812In this one, I rant about why holding onto the wrong tasks is quietly wrecking clinic owners' businesses, using the mechanic phone-tag story and a mother-and-ironing analogy to make the point.Uncovering the real reason business owners keep holding onto low-value work — and what it's secretly costing themBreaking down a simple test to spot where your time is misallocated before it turns into a much bigger problem
811. EP 811 🔥 "Is Your Comfort Zone Costing You Six Figures?" [Paul on Fire]
08:37||Ep. 811Today, I break down why avoiding marketing and outreach is quietly bankrupting clinic owners, and why workshops are outperforming digital ads right now.Explained that success comes down to a binary choice — go do the "ground game" (networking, referrals, in-person relationships) or master direct marketing (Google, Facebook, email) — and that avoiding both out of fear or comfort is what's actually costing owners moneyBroke down why monthly in-person workshops are now beating Google ads, Facebook ads, and social DMs for driving new patients, using real data from 350+ consulted clinics and the AI-driven "information overload" reshaping how patients make decisions
810. EP 810 ☕️ "Are the PTs Asking for 150K Actually Any Good?" [Coffee with Goughy]
01:11:46||Ep. 810Today, I made the case that patients don't pay for research or clinical credentials — they pay for "the magic," and that's the real litmus test for whether a PT is worth what they're asking for.Introduced two litmus tests for a PT's true value: how many patients specifically call and ask for them by name, and how much of a fuss gets kicked up when you try to move them to another providerArgued that devotion, communication, and basic human courtesy (eye contact, please and thank you) build a practice far more than clinical research ever will, using the barbershop/loyalty analogy to make the pointShared the story of growing his own caseload from 15 to 71 patients through sheer devotion and desire, with no marketing systems at all, to illustrate what separates a great PT from an average one
809. EP 809 ☕️ "Why Don't My Staff PTs Respect Me?" [Coffee with Goughy]
01:13:52||Ep. 809Today, I tackled the question "Why don't my staff PTs respect me?" — tracing it back to a self-respect problem in the business owner rather than the employees themselves.Explained why respect from staff starts with an owner's own self-belief, and broke down the cultural shift since 2005 where employees stopped automatically respecting "the boss"Unpacked how entitled, "why should I?" attitudes trace back to expensive degrees, indoctrinating colleges, and a generation raised without real risk or consequencesArgued that being an employee doesn't entitle you to the upside of risks you didn't take, and made the case for why owners need unshakeable self-belief to run a successful business
