EP697 Paul Meets: Allison Feldt | From New Mom to Multi-Million Dollar Clinic Owner
In this episode of Paul Meets, I sit down with someone I’ve known since the very early days of her journey, Allison Feldt (https://bodymotionpt.com/).
Allison’s story is one of the most important conversations I’ve had on this podcast.
She started her business in 2017 just weeks after having her second child. No safety net. No guaranteed income. No clear plan. Just a deep need to make something work for her family and a willingness to learn what most clinic owners avoid.
Fast forward to today and Allison runs a multi-million-dollar physical therapy business with a 17-person team, has fully stepped out of treatment, and is now building something much bigger than a clinic.
In this conversation, we talk about:
- What it actually feels like trying to get from $100K to $300K when you’re stressed, overwhelmed, and carrying everything
- Why learning marketing and sales changed not just her income, but patient outcomes
- The guilt, resistance, and outside voices that tell you to “just be grateful” and stop wanting more
- How unresolved personal baggage quietly caps your business growth
- The identity shifts required to move from clinician to leader to brand
- Why stepping out of the clinic was harder emotionally than financially
- How personal brand, visibility, and being “seen” became the real growth lever
- Why the future of PT is not just healthcare, but healthcare plus wellness and media
This isn’t a tactics episode.
It’s a conversation about identity, leadership, confidence, motherhood, ambition, and what it really takes to build a business that doesn’t trap you.
If you’re a clinic owner who’s been told you have to choose between family and success, or that your ambition should quiet down once life gets busy, this episode will challenge that belief head-on.
In this episode of Paul on Fire, I’m laying out five predictions I’m extremely confident about for 2026 and why the “middle of the road” clinic is about to get crushed. If your practice is sitting in that uncomfortable $20K–$40K per month zone with decent marketing, rising costs, and shrinking margins, this is your warning. The clinic itself is no longer the asset. Location is no longer the advantage. Visibility is.
I break down why attention will matter more than bricks and mortar, why “good enough” marketing won’t be good enough anymore, and why clinics with authority, leverage, and systems will pull away fast. I also explain what’s really happening on Facebook and Instagram right now, why authentic insight will outperform polished content, and why this is a rare 12–18 month window you cannot afford to waste.
We also talk AI. Not as a shiny tech toy, but as an operational survival decision. AI won’t replace physios, but it will replace inefficient clinics, front desk chaos, bloated payrolls, and slow follow up. Clinics that ignore it will feel slower, more expensive, and more stressful in 2026.
Finally, I get into the real bottleneck most clinic owners won’t want to hear about: identity. The way you see yourself will either be your biggest accelerator or the thing that quietly holds you back. This episode is about leadership, leverage, visibility, and making the shift before the market forces it on you.
If you want a real wake up call heading into 2026, this one’s for you.

09:08||Ep. 759In this fired-up rant, I break down what it really means to become a "unique self-creation" — and why your tattoos, car, and haircut don't count. I challenge the idea that following society's rules about marriage, routines, and success will ever make you happy, using the classic fisherman parable to drive it home. It's a wake-up call to stop living by everyone else's blueprint and start designing a life that makes you say "I wasn't supposed to do this."

17:39||Ep. 758In this episode, I go deep on something that most clinic owners get completely wrong - what your patients are actually paying you for. Spoiler: it's not the treatment.I get asked:"Why will a patient pay you $350 when they can get the same treatment for $150 around the corner?""What did Mr. Gucci understand about his customers that most clinic owners completely miss?""Why do some clients disappear for nine months, never use a single thing you offer, and still happily keep paying you?""How did the UCLA "See I Care" model completely change the way I think about patient experience?""What's the difference between good customer service and the kind of experience people will pay a premium for?""How do you know when your business is built to last — and how do you know when it's in trouble?"Register Now: https://cashpaysummit.com/

15:27||Ep. 755Insurance-based clinic owners are calling me reckless. They say cash pay is a fad. So I decided to answer them live on air. Here's what gets thrown at me and how I answered:Insurance-based clinic owners are calling you reckless - they say cash pay is a fad. What do you say to their face?People are calling your cash model a scandal - charging patients directly and cutting out insurance. 745. EP745 ☕"I Missed My Baby's First Year Building The Clinic. Was I Wrong?" [Coffee With Goughy]
01:12:28||Ep. 745On this episode of Coffee with Goughy, we go over some comments left on my social media:The viral post that sparked massive debate — "I Missed My Baby's First Year Building The Clinic. Was I Wrong?"Why people react so emotionally and quickly when I question how we think about parenting on social mediaThe comment that said "You have the rest of your life to make money — you can never get that time back" and why I disagreeThe commenter who called my perspective "atrocious" and the person who fired back with "That's why you'll never make it — you wouldn't date a broke man"Why people view money and family as an either/or choice instead of figuring out how to have bothThe difference between physical presence and emotional presence with your childrenWhy I believe 45 minutes of fully conscious time with your kid beats 4 hours of distracted, phone-addicted "quality time"The hypocrisy of people who judged me for working late with my first child — then told me "You're lucky because you've got the money" when my fifth child came alongWhy I think parenting is the last thing insecure people cling to as proof they're a success — and why that makes it such a hot-button topicThe difference between good guilt and bad guilt, and how I use guilt as fuel instead of letting it hold me backRegister Now: https://cashpaysummit.com/

18:05||Ep. 744In this episode, I sit down with an insurance-based clinic owner who's running at 90% insurance and 10% cash pay. He wants to shift gears but he's scared. He doesn't know how to make the transition without blowing up what he's already built. Here's what gets asked and answered:How do we shift from 90% insurance to more cash pay without risking the business we've already built?Do I need to close the doors on insurance and start fresh — or is there a smarter way?How do I turn a $100-per-session insurance patient into a $2,000–$3,000 lifetime customer? What's the difference between taking risks and managing risks — and why does that matter for this transition?5. How do I get patients to buy more when they don't even know what else I offer? How long does this realistically take — and why do most clinic owners get the timeline wrong? How do I run multiple things at once without losing my mind?Register Now: https://cashpaysummit.com/
