During the 8th International Chronic Graft-versus-Host Disease Symposium, the GvHD Hub held a symposium on May 22, 2026, titled Evolving treatment strategies in SR‑cGvHD: Optimizing patient selection across current and emerging therapies. Here, we share a presentation by Corey Cutler, Dana-Farber Cancer Institute, Boston, US, describing the impact of clinical manifestations on treatment selection in steroid-refractory chronic graft-versus-host disease (SR-cGvHD). Cutler provides an overview of the inflammatory and fibrotic mechanisms underlying cGvHD, including the roles of the JAK1/2, ROCK2, and CSF-1R signaling pathways. He outlines the heterogeneous, organ-specific responses observed with targeted therapies and highlights that treatment selection may be informed by consideration of expected organ-specific responses and disease phenotype, with growing efforts to target the underlying biological mechanisms of cGvHD.  This educational resource is independently supported by Sanofi. The GvHD Hub spoke with Corey Cutler, Dana-Farber Cancer Institute, Boston, US. We asked, How do you approach later-line sequencing in cGvHD in your clinical practice?  Cutler discusses his approach to selecting second-line and subsequent therapies for patients with cGvHD. He explains that, in his practice, treatment selection is guided primarily by FDA approval status and toxicity profiles, while organ involvement plays a more limited, context-dependent role. He highlights patient comorbidities and practical considerations, such as route of administration and dosing frequency, as additional factors that may influence therapy selection. The GvHD Hub spoke with Robert Zeiser, University of Freiburg, Freiburg, DE. We asked, How might emerging therapies and combination approaches reshape sequencing strategies in cGvHD? During this interview, Zeiser discusses emerging treatment strategies under evaluation in cGvHD, including the latest clinical data for several combination approaches and novel agents. During the 52nd Annual Meeting of the EBMT, the GvHD Hub held a symposium on March 24, 2026, titled “Focusing GvHD care through the eyes of providers and patients: Practical insights on ECP”. Here, we share a presentation by Matea Meden, describing her experience with GvHD. Matea describes her journey with GvHD, focusing on its profound impact on her physical health, emotional wellbeing, and overall quality of life. She outlines her treatment pathway, which included steroids, ruxolitinib, and extracorporeal photopheresis (ECP). She highlights both the practical and physical challenges of ECP alongside the gradual clinical improvements she experienced. Matea also shares advice for patients with GvHD, encouraging them to trust the ECP treatment process, communicate openly with their healthcare team, and draw strength from their family, peers, and support networks. During the 52nd Annual Meeting of the EBMT, the GvHD Hub held a symposium on March 24, 2026, titled “Focusing GvHD care through the eyes of providers and patients: Practical insights on ECP”. Here, we share a presentation by Hildegard Greinix, Vienna, AT, describing her approach to treating GvHD with ECP. Greinix presents two patient case studies illustrating the use of ECP for the management of both acute and chronic GvHD. In both cases, she notes that ECP could be considered a second‑line or later treatment option, either as monotherapy or in combination with other agents, due to its immunomodulatory mechanism of action and favorable safety profile.
