The answer, he argues, lies in the relationship between large healthcare organizations and insurance companies — a system that decides what gets paid for, what gets denied, and what care you actually receive. Clinicians who know you need A, B, and C are forced to give you X, Y, and Z because that's what insurance reimburses.
