Con motivo del <strong>Día Mundial del X Frágil</strong>, conversamos con el Dr.&nbsp;<strong>Luis Ernesto Marfil Marín</strong>, médico genetista con alta especialidad en neurogenética en el Hospital Psiquiátrico Infantil Juan N. Navarro, para entender a fondo esta condición genética.</p><p><br></p><p>El síndrome X frágil es una condición genética del neurodesarrollo y la causa hereditaria más frecuente de&nbsp;<strong>discapacidad intelectual</strong>. Se origina por una&nbsp;<strong>expansión anormal de repeticiones CGG</strong>&nbsp;en el gen&nbsp;<em>FMR1</em>. Cuando estas repeticiones superan las doscientas, se producen la&nbsp;<strong>metilación</strong>&nbsp;y el silenciamiento del gen, lo que impide la producción de una proteína necesaria para la plasticidad sináptica y el desarrollo del sistema nervioso central. Clínicamente, los pacientes presentan&nbsp;<strong>retraso global del desarrollo</strong>,&nbsp;<strong>trastorno del espectro autista</strong>,&nbsp;<strong>TDAH</strong>, ansiedad e hiperactividad. Aunque existen ciertas características físicas asociadas, como cara alargada y orejas prominentes, el diagnóstico no se basa únicamente en la clínica: requiere&nbsp;<strong>pruebas moleculares específicas</strong>&nbsp;, como la&nbsp;<strong>PCR</strong>&nbsp;y el&nbsp;<strong>Southern blot</strong>&nbsp;, para confirmar la expansión y el estado de metilación. En el manejo, aunque no existe una cura, la&nbsp;<strong>intervención temprana</strong>&nbsp;importa desde el inicio. El abordaje incluye&nbsp;<strong>terapia del lenguaje</strong>,&nbsp;<strong>terapia cognitivo-conductual</strong>&nbsp;y&nbsp;<strong>terapia ABA</strong>, que pueden apoyar el aprendizaje y la conducta.
